AHA Book Request Form

INSTRUCTIONS

Please complete and submit the following form to request an American Heart Association Provider Manual for your required courses. Requests for manuals must be placed 1 week in advance to ensure timely delivery. 

LOAN PERIOD AND RETURNS

The standard loan period for BLS, ACLS, and PALS is 3 weeks, with 2 renewals allowed per loan. For NRP and STABLE, and Fetal heart monitoring, the standard loan period is 30 days. Books can be returned by placing them in the book dropboxes available at each of the Lee Health Medical Libraries. A list of our locations is available on the Medical Library website.

LATE FEES, DAMAGES, AND REPLACEMENT

Library users are responsible for the resources they borrow. Lost and damaged books will result in a replacement fee. Late returns will incur a fee of $.25/day, up to a maximum of $10.00. Books not returned after 10 days will automatically be charged as lost, with late fees being replaced by the full cost of the book
